yellowdotPennDOT created the Yellow Dot program to allow drivers to share written medical and personal information with first responders in the event of an emergency.

Participants affix a PennDOT-provided Yellow Dot decal to the lower left corner of the rear window of their vehicles. This sticker alerts emergency responders to the availability of information to help them provide better care to crash victims. In conjunction with these decals, participants complete an official Yellow Dot form, which contains information including an emergency contact, medical conditions and prescriptions. The form also provides space to attach a small photo of the corresponding participant.

Yellow Dot participants complete a simple questionnaire with information on their medical condition, medications, allergies and emergency contacts, and place the questionnaire in a yellow folder in their vehicle’s glove box. A yellow sticker is then affixed to the rear vehicle window to alert emergency responders that the driver participates in the program. The program is free to all Pennsylvanians.
